Back to Balance, Inc is currently a fee-for-service/self pay practice, and payment for services is due at time of visit. For financial assistants, we offer discounted rates and installment plans if needed. We accept cash, credit cards through online payments, and checks made payable to Back to Balance, Inc. We can provide you with the necessary reimbursement forms, containing accurate diagnosit and service codes in order to file with your insurance carrier for reimbursement.
A minimum of 24 hours is required for cancellation or rescheduling all appointments. You will be charged a $50 fee for same day cancellations. If you had a family or medical emergency, however, please let us know.
